Worst Restaurant Experience

I have visited this restaurant in the past and was satisfied. However, we were recently there (last week) and received the worst service ever.
These are the issues we experienced which made up my mind that we will never go back. First, we walk in at noon and it is not busy. There are 5 of us, two parents, and 3 children (13,7,and 5).
